Forms must be submitted at least two (2) weeks in advance. Some requests may require a thirty (30) day advance notice, depending on the nature of the activity.
Please complete the form in its entirety. This will save time and guess work in processing the request.
Attach a diagram if a special set-up is needed (i.e. auditorium style, specific directions, semi-circle, etc.)
Pay close attention to the furnishings/equipment needed section of the form specifying quantity of chairs, tables, type of tables (round or rectangular) and chairs and other special needs. Please be specific.
If media support is needed, see audio/visual instructions. Please note, media thirty (30) days notice.
The Ministry Director and the Pastor must approve the form.
After completing the above steps, the form will be submitted to the Church Office.
In the event that an area is not available, you will be contacted by the Facilities Events Coordinator.
